Manie
Unter einer Manie vesteht man eine meist episodisch verlaufende, länger anhaltende, der Situation unangemessene Stimmungs- und Verhaltensänderung, typischerweise im Sinne einer übertriebenen Heiterkeit, Überdrehtheit und Euphorie, im Verbund mit Distanzlosigkeit bis Schamlosigkeit, Selbstüberschätzung und verminderter Kritikfähigkeit. Der Antrieb während einer Manie ist gesteigert bis hin zu unkontrollierter Erregbarkeit. Dies macht sich bemerkbar in einer Beschleunigung des Redens und Denkens, in einem vermehrten Bewegungsdrang und einer meist erheblich gesteigerten Entschlussfreudigkeit. Gelegentlich ist in der Manie auch eine vermehrte Gereiztheit und provokant-aggressives Verhalten vorhanden. Meist herrscht ein Gefühl besonderen körperlichen und geistigen Wohlbefindens und gesteigerter Leistungsfähigkeit. Auch die sexuelle Lust ist oft gesteigert. Bei schweren Formen einer Manie kann es zu massiven Grössenphantasien und vorübergehenden psychotischen Realitätsverkennungen kommen, die sich nach dem Abklingen der manischen Phase wieder vollständig zurückbilden.
In der Sprache der Psychopathologie sind die typischen, diagnostisch wegweisenden Symptome der Manie:
- Antriebssteigerung
- Grössenideen
- Ideenflucht
Ein stark reduziertes Schlafbedürfnis gilt bei Betroffenen als besonderes Frühwarnsymptom für eine bevorstehende manische Phase.
Für die Diagnose einer Manie nach ICD-10 müssen die Symptome mindestens 1 Woche bestehen und so ausgeprägt sein, dass berufliche und soziale Beeinträchtigungen vorhanden sind.
Die Manie tritt nur in 6% als eigenständige Erkrankung, d. h. ohne vorausgehende oder nachfolgende depressive Phasen auf, in 28% ist sie Teil der sogenannten „bipolaren Störung“ (manisch-depressive Krankheit).
