Zur Therapie der schizoaffektiven Störung werden vorrangig hoch- und niederpotente Neuroleptika eingesetzt. Das gilt sowohl für die schizodepressive Störung, als auch für die schizomanische Störung. Oft bilden sich bei Ansprechen der neuroleptischen Therapie die affektiven Symptome (Depression und Manie) von selber zurück. Wo dies nicht gelingt, kann die schizomanische Störung zusätzlich mit einem Antimanikum bzw. einem Phasenprophylaktikum (z. B. Lithium) und die schizodepressive Störung mit einem Antidepressivum behandelt werden.
