Ängste gehören zum Leben, wie Gefühle überhaupt. Sie treten in den unterschiedlichsten Situationen und Zusammenhängen auf, weisen auf drohende Gefahren hin und haben damit eine wichtige Schutzfunktion für den Menschen.

Berichtet ein Patient über zunehmende Ängste, so gehört die Klärung der Frage, ob es sich um eine „Angststörung“ oder eine „berechtigte Angst“ handelt zu den allerersten und wichtigsten Massnahmen. Denn der Blick kann verstellt sein dafür, dass eine aufkommende Angst auf eine tatsächlich bestehende Gefahr hinweisen könnte (z.B. auf das Entgleiten eines Lebenskonzeptes, eine schwelende Beziehungskrise etc..) oder, wie im Fall der Phobie, eine möglicherweise berechtigte Angst der psychischen Abwehr auf einen unbedeutend scheinenden Gegenstand verschoben sein könnte oder sich wie bei den Panikattacken in verfremdeter Form am falschen Ort zeigen und in ihrer ursprünglichen Bedeutung dadurch nicht direkt zu erkennen sein könnte. Angst kann auch ein „Begleitsymptom“ einer anderen Erkrankung (körperlich oder psychisch) sein, weshalb das Auftreten einer solchen Symptomatik eine umfangreiche medizinische und psychiatrische Abklärung notwendig macht.In solchen Fällen einer eigentlich berechtigten Angst wäre es kurzsichtig, diesen Affekt sofort zum (einzigen) „Symptom“ zu erklären und entsprechend „symptomatisch“ zu behandeln. Vielmehr muss geprüft werden, ob nicht z.B. die „Ahnungslosigkeit“ des Patienten, woher seine Angst kommen könnte, das eigentliche Symptom darstellt und im Zentrum der weiteren Diagnostik und Therapie zu stehen hätte. Meist geht in der Praxis dann beides Hand ihn Hand und die konkrete Behandlung gestaltet sich dann eben „mehrdimensional“ und vor allem möglichst „kausal“, also auf die Ursache bezogen. Häufig sind allerdings Ängste und Panikattacken tatsächlich Störungen im eigentlichen Sinne des Wortes, z.B. ein angeborener Überschuss an Angst oder eine (möglicherweise durch biographische Faktoren mitbedingte) Irritation oder Fehlentwicklung im affektiven Bereich, die dann gezielt angegangen, mit kognitiven und übenden Verfahren sowie medikamentöser Unterstützung in Grenzen gehalten bzw. möglichst überwunden werden müssen. Ein effektives Training ist dann sehr wichtig, damit der Betroffene nicht stetig in seiner Lebensentfaltung behindert und im Verlauf invalidisiert wird.

Phobien

„Phobien“ sind irrationale Ängste vor ganz bestimmten Dingen oder Situationen, die sich durch Vermeidung des Auslösers kontrollieren lassen. Dem betreffenden Patienten ist meist bewusst, dass seine Ängste übertrieben sind. Ziel einer phobischen Reaktion können die unterschiedlichsten Dinge sein. Häufig ist die sogenannte soziale Phobie, d. h. das Auftreten starker Ängste bis hin zur Panik in sozialen Situationen. Weitere bekannte Phobien sind die Flugangst, die Spinnenangst oder Spinnenphobie (siehe unten), die Platzangst und die Redeangst.

Phobien betreffen auch häufig das Aufhalten in grossen Menschenmengen , das Einkaufen in grossen Kaufhäusern, das Überqueren grosser Plätze, das Autofahren, das Durchfahren von Tunneln, das Benutzen eines Aufzugs oder der U-Bahn (Metro), eines Busses oder Zuges.

Häufig ist die sogenannte „Agoraphobie„, die sich ursprünglich nur auf die Angst vor grossen öffentlichen Plätzen bezog und heute aber andere typische Ängste mit einbezieht, so etwa generell die Angst vor anderen öffentlichen Situationen, wie das Aufhalten in Menschenmengen, Angst vor dem Busfahren, vor Reisen oder überhaupt vor dem Verlassen des Hauses oder des Wohnortes.

Es gibt eine Unzahl von möglichen Phobien, phobische Reaktionen können sich auf alles und jedes richten. Bei ganz umschriebenen phobischen Ängsten nennt man dies eine spezifische Phobie. Im Folgenden sind einige bekanntere spezifische Phobien aufgelistet:

  • Angst vor Katzen (Ailurophobie)
  • Angst vor Vögeln (Ornithophobie)
  • Angst vor Spinnen (Arachnophobie)
  • Ängste vor engen Räumen (Klaustrophobie)
  • Ängste vor Blut (Hematophobie)
  • Ängste vor grossen Höhen (Bathophobie)
  • Angst vor dem Fliegen (Flugangst, Aviophobie)
  • Angst vor Krankheiten (z.B. Krebs)
  • Angst vor grossen Menschenmengen (Demophobie, Ochlophobie) )
  • Angst vor grossen Räumen, z. B. auch Angst in grossen Kaufhäusern
  • Angst vor dem Sprechen (Glossophobie)
  • Angst, in der Öffentlichkeit das Wort zu ergreifen (Homilophobie)
  • Angst vor dem Auftreten der Angst („Angst vor der Angst“, Phobophobie)

Über das Gefühl der Angst:
Der Psychiater und Philosoph Karl Jaspers schrieb im Unterkapitel „Gefühle und Gemütszustände“ seiner berühmten Allgemeinen Psychopathologie (9. Aufl. 1973) über die Angst:“

  1. Ein häufiges und qualvolles Gefühl ist die Angst. Furcht ist auf etwas gerichtet, Angst ist gegenstandslos. Als eine spezifische Gefühlsempfindung des Herzens ist die Angst vital, unterscheidbar als stenokardische Angst (bei Angina pectoris) und als Erstickungsangst (bei Lufthunger, z.B. dekompensierten Kreislaufstörungen). Aber Angst ist auch ein ursprünglicher Seelenzustand, in Analogie zur vitalen Angst immer das Dasein im ganzen betreffend, es durchdringend und beherrschend. Von einer inhaltlosen gewaltigen Angst, die zur Trübung des Bewusstseins und rücksichtslosen Gewaltakten gegen sich selbst und andere führt, bis zur leichten, als fremd und unverständlich empfundenen Ängstlichkeit gibt es alle Grade. Angst ist mit körperlichen Sensationen, Druck-, Erstickungs-, Engegefühl verbunden. Oft wird sie lokalisiert, z.B. als Präkordialangst, manchmal sogar als Kopfangst. Ein Kranker meinte, er habe den Drang, körperlich hineinzufahren, wie mit dem Zahnstocher in einen schmerzenden Zahn. Die existentielle Angst, eine Grundverfassung des sich in Grenzsituationen offenbar werdenden Daseins, dieser Ursprung der Existenz, ist phänomenologisch nicht mehr fasslich.
  2. Mit der Angst ist meistens ein lebhaftes Gefühl der Unruhe verbunden. Dieser Gemütszustand innerer Erregung kann auch ohne Angst isoliert vorkommen. Als „Nervenerregung“, als „Fieber“ werden diese Gefühle nachher von den Kranken bezeichnet. In leichteren Graden tritt der Zustand als Gefühl, noch etwas tun zu müssen, etwas noch nicht fertig zu haben, als Gefühl des Suchens, des Klarwerdenmögens auf. Bei erlebnisreichen Psychosen steigert sich das Gefühl der Unruhe zu Spannung und Gepresstheit, zu einem nicht mehr Aushaltenkönnen der massenhaften Eindrücke, so dass nur eines begehrt wird: Ablenkung und Ruhe.“ (S. 95)

Obwohl Angst ein „Affekt“ ist, werden Angststörungen, Panikattacken und auch phobische Störungen in der ICD-10, einem modernen Klassifikationssystem, nicht in der Rubrik F3 (Affektive Störungen), sondern im Kapitel F4 (Neurotische-, Belastungs- und somatoforme Störungen) kodiert.Menschen mit Angststörungen empfinden selber oft gar nicht die Angst als das hervorstechende Symptom. Stattdessen werden häufig körperliche Symptome, wie etwa Schwindel, Herzrasen, Zittern, verminderte Belastbarkeit oder auch Magen-Darm-Beschwerden zuerst genannt. Wenn gar kein Bezug zwischen den körperlichen Symptomen und einer dahinterliegenden Angst vom Patienten hergestellt werden kann und eine solche Beziehung aller Wahrscheinlichkeit nach doch besteht, dann spricht man bei den körperlichen Symptomen von „Angstäquivalenten“. Die Symptome stehen sozusagen für die Angst.Die Folgen von Angststörungen sind mitunter gravierend: Weil man sich schämt, die Ängste einzugestehen und Hilfe zu holen, wird das Problem vor anderen und oft sogar vor sich selber verleugnet. Angstauslösende Situationen werden gemieden, das sonstige Leben um die Ängste herum neu organisiert, dies alles auf zunehmend unrichtiger Argumentationsbasis. Die weitere Folge ist oft eine soziale Isolierung sowie ein Zurückbleiben hinter den eigenen Möglichkeiten bei der Übernahme gesellschaftlicher Verantwortung und im Beruf. Steigt der Druck, werden häufig Alkohol, Beruhigungs- und Schlafmittel oder, v.a. bei jungen Patienten Drogen konsumiert, um sich zu betäuben. Daraus entstehen schnell zusätzliche Abhängigkeitserkrankungen und in der Folge auch depressive Entwicklungen.

Weil Ängste im Zusammenhang mit jeder psychiatrischen Erkrankung auftreten können, ist die Diagnosestellung nicht immer einfach, und es gibt sowohl eine hohe Komorbidität, als auch eine starke Kriterienüberlappung zu anderen Störungsbildern, z.B. zu Persönlichkeitsstörungen, Depressionen, Dysthymien etc.. Dies wirft Fragen bezüglich der Güte der aktuellen Konzeptionalisierungen auf, auf die auch in der Literatur hingewiesen wird (z.B. Kapfhammer, 2000).

Typische Symptomatik bei Generalisierter Angststörung, Panikattacken, sozialer Phobie und Agoraphobie

Symptomatik bei generalisierter Angststörung (frühere Bezeichnung: „Angstneurose“):

  • andauernder ängstlich-angespannter Zustand
  • andauernde Besorgnis alltäglichen Situationen und Ereignissen gegenüber
  • Katastrophenbefürchtungen ohne besonderen erkennbaren Grund („Meinem Sohn könnte etwas schlimmes zustossen“)
  • übergrosse Nervosität und leichte Schreckhaftigkeit
  • Fixierung des Denkens auf erlebte oder mögliche Katastrophen und Bedrohungen
  • Schlafstörungen
  • Vermeidung angstverstärkender Situationen
  • Rücken-, Nacken- und/oder Kopfschmerzen aufgrund eines ständig erhöhten Muskeltonus
  • Tinnitus
  • Übelkeit
  • Verdauungsprobleme
  • Trockener Mund

Typische Symptomatik einer Panikattacke (frühere Bezeichnung: „Herzphobie“):

  • wiederholte abgrenzbare Panikattacken, die unerwartet und nicht an bestimmte Situationen gebunden auftreten
  • Dauer ca 10 – 30 Minuten, aber auch bis zu mehreren Stunden
  • ausgeprägte Erwartungsangst (Angst vor der Angst = Phobophobie!)
  • starke Beklemmungsgefühle
  • intensive vegetative Begleitreaktionen (Herzrasen, Atemnot, Druckgefühl im Thorax (Angst vor Herzinfarkt), Ohnmachtsgefühle Todesangst oder Angst vor Kontrollverlust, Schwitzen, feuchte, zittrige Hände, Übelkeit etc.)
  • häufig gleichzeitig bestehende Agoraphobie

Typische Symptomatik einer sozialen Phobie:

  • starke Ängste, sich in bestimmten sozialen Kontexten zu zeigen
  • extreme Angst, in dieser Angst erkannt und öffentlich beschämt zu werden
  • vor und in angstbesetzten Situationen starke körperliche Reaktionen (Herzrasen, Schwitzen, Atemnot, Stimmversagen u.a.)
  • starkes Vermeidungsverhalten, dadurch oft ausgeprägtere Defizite beim „normalen“ Reifungsprozess und Defizite bei der Wahrnehmung sozialer Verantwortung
  • häufig in der Folge sehr schlechtes Selbstbewusstsein, Versagensgefühle, Unterlegenheitsgefühle, Furcht vor Kritik
  • Erröten, Zittern der Hände, Vermeidung von Blickkontakt, Übelkeit, auch Harndrang

Typische Symptomatik der Agoraphobie:

  • heftige Angst, sich nicht rechtzeitig in Sicherheit bringen zu können, deshalb keine grössere Entfernung von „sicheren Orten“ oder wichtigen Personen
  • Angst v.a. in grossen Menschenmengen
  • Angst vor öffentlichen Plätzen
  • Angst, auf Reisen alleine zu sein oder sich zu weit von zuhause zu entfernen
  • starke Vermeidungstendenz mit zunehmender Einengung des Aktionsradius. In schweren Fällen kann die eigene Wohnung nicht mehr verlassen werden
