Wie und wodurch entstehen die Alzheimer Demenz und andere Demenz-Formen?

Ursachen für dementielle Prozesse (nach Möller 2001)

  • Hirndurchblutungsstörungen (vaskuläre Demenz)
  • primär degenerative Erkrankungen der Hirnrinde (Alzheimer-Krankheit, Picksche-Krankheit)
  • Defekte im Marklager (z.B. subkortikale Dystrophie Seitelberger oder die progressive supranukleäre Blicklähmung)
  • Infektionen des Zentralnervensystems (z.B. Creutzfeldt-Jakob-Erkrankung, Enzephalitis)
  • sogenannte Systematrophien (Morbus Parkinson, Chorea Huntington)
  • Störungen der Liquorzirkulation (Normaldruck-Hydrozephalus (NPH))
  • Vergiftungen (Intoxikationen) durch Medikamente, Schwermetalle, Kohlenmonoxid, Alkohol, organische Lösungsmittel
  • Hirntumoren, Hirnmetastasen oder paraneoplastische Prozesse
  • Vitaminmangelzustände (Vit.-B-12-Mangel (Perniziosa), Vit.-B1-Mangel (Thiaminmangel), Folsäuremangel, Nikotinsäuremangel (Pellagra)),hormonelle und Stoffewechselerkrankungen,
  • Hirnverletzungen (Blutungen, Erschütterungen)
Ursache und Entstehung der Alzheimer Demenz
Ursache ( Ätiologie)

Die Ätiologie der Alzheimer-Demenz ist nur zum Teil geklärt. Folgende Faktoren spielen eine ursächliche Rolle:

  • in 30% d. Fälle sind genetische Veränderungen die Ursache der Alzheimer Demenz. Es handelt sich um Punktmutationen auf den Chromosomen 1, 14 oder 21, die trotz der unterschiedlichen Lokalisation zu denselben pathogenetischen Endstrecken führen, nämlich zu einer vermehrten Ablagerung von „Amyloid“, einem pathologischen Eiweiß, im Gehirn.
    Man hat ausserdem gefunden, dass Menschen, auf deren Chromosom 19 das Gen für Apolipoprotein E (ApoE) in einer bestimmten Zustandsform vorliegt (als „ApoE-epsilon4-Allel„), überdurchschnittlich häufig an Alzheimer erkranken. Diese Zustandsform in sich zu tragen wird als „Risikofaktor“ gewertet, ist aber eigentlich eine „Normvariante“ Das bedeutet, das blosse Vorhandensein dieses Allels kann für sich genommen die Erkrankung nicht
  • Weitere, nicht-genetische Risikofaktoren sind
    • positive Familienanamnese (d.h. Erkrankungsfälle von) Demenz, Parkinson-Krankheit, Down-Syndrom
    • Hirnvorschädigungen, Depressionen, Schilddrüsenunterfunktion
Entstehung (Pathogenese)

Der Entstehungsmechanismus ist unabhängig von der zugrundeliegenden Ursache gleich: es kommt, meist über Jahrzehnte, zu einem neurodegenerativen Prozess, d.h. zu einem Abbau an Hirnsubstanz, die häufig in bestimmter Reihenfolge bestimmte Hirnregionen, schwerpunktmässig den Schläfenlappen(Temporallappen) und den Parietallappen betreffen. Eine besondere Bedeutung spielt der Befall der „Hioppocamus-Region“. Viele Jahre nach der beidseitigen Schädigung dieser Hirnregion kommt es zu den ersten neuropsychologischen Defiziten im Bereich von Lernen und Gedächtnis, dann zu Änderungen der Affekte (z.B. depressiver Rückzug).

Im Rahmen des neurodegenerativen Prozesses kommt es zur Ausbildung der beidenpathognomonischen Kennzeichen der Alzheimer-Demenz, die sich erst nach dem Tod in der Hirnbiopsie nachweisen lassen, zur Ablagerung von

  • „Amyloid„, sogenannten „Plaques“, die man unter dem Mikroskop erkennen kann
  • „Neurofibrillenbündel„

Die ersten Symptome treten erst relativ spät auf, wenn der Prozess schon erheblich fortgeschritten ist.
